Organ Donor Gift of Life Program

If you are at least 18 years old, you may designate on your driver license or ID card that you wish to participate in the gift of life program. The DMV can provide you with a donor card (DL 290) and sticker. The donor sticker is placed on the front of your driver license or ID card. Donors should share their decision with family members.

Online Registration for the Organ and Tissue Registry. You can sign up online to be an organ and tissue donor.
