Information Security Statement (INF 1128 Rev. 09/2006)

The INF 1128 form (PDF):

• must be signed by any persons who have access to department information
• must be completed and retained at the worksite of the CRA Holder.
• Must be re-certified annually

Forms must be:

• printed and completed by hand, or
• printed and completed by typewriter, then signed
• completed on-line, printed, and signed.
