Information Security Statement (INF 1128 Rev. 09/2006)
The INF 1128 form (PDF):
- must be signed by any persons who have access to department information
- must be completed and retained at the worksite of the CRA Holder.
- Must be re-certified annually
Forms must be:
- printed and completed by hand, or
- printed and completed by typewriter, then signed
- completed on-line, printed, and signed.
