Commercial Requester Account Surety Bond (INF 1132 Rev. 9/2004)

The INF 1132 form (PDF):

• is an Agreement between the Surety and the Principal
• is an Agreement to pay the DMV $50,000 in the event of misuse
• must be completed and submitted only if the CRA holder is approved for residence address access to department information.

APU cannot accept forms submitted by e-mail at this time. Forms must be:

• printed and completed by hand, or
• printed and completed by typewriter, or
• completed on-line and printed.
