- KEEP COPIES OF ALL FORMS FOR YOUR RECORDS PRIOR TO SUBMITTING THEM TO THE DMV. COPIES WILL NOT BE RETURNED

- printed and completed by hand, or
- printed and completed by typewriter, or
- completed on-line and printed.
