Request for Vehicle/Vessel Automated Record Information (INF 1123)
The INF 1123 form:
- is used when a Commercial Requester Account Re-Seller submits a request for vehicle/vessel information from the Department of Motor Vehicles (DMV) on behalf of another approved Requester Account End-User.
- must contain the Commercial Requester Account Re-Seller Requester Code, and Agreement numbers issued by the DMV.
- must contain the Commercial Requester Account End-User’s Requester Code and Agreement numbers issued by the DMV.
- must be submitted in duplicate.
- must be submitted to
the address on the form or the address listed below.
DMV cannot accept forms submitted by e-mail at this time. Forms must be:
- printed and completed by hand, or
- printed and completed by typewriter, or
- completed on-line and
printed.
