Application for Duplicate Title (REG 227)

• A Duplicate Title
• Paperless Title Certification
• Transfer of Title With Duplicate Title
• Transfer of Title With Paperless Title

You may submit this application either in person or by mail. You can make an appointment at your local DMV or mail the completed forms with payment to the following address. Additional information regarding duplicate titles.
