Organ and Tissue Donor Information PDF Version
New Policy
New legislation requires the Department of Motor Vehicles to ask on the driver license application form if the applicant would like to be an organ and tissue donor, and/or to donate $2 to Donate Life California to promote organ and tissue donation.
Changes to DL 44
The revised DL 44 includes a space for the applicant to indicate if he/she wishes to be an organ and tissue donor, and/or would like to make a monetary donation to Donate Life California.
Procedures
On July 1, 2006, the revised DL 44 application form must be used.
- Continue to use existing DL 44 application forms until June 30, 2006.
- Recycle all previous versions of the DL 44 on July 1, 2006.
- Order the new revised DL 44 before July 1, 2006.
Background
Currently, the department issues organ donor stickers that are affixed to the DL/ID card, however, many stickers come off and there was no registry available that had updated donor information. New legislation provides for an embedded donor indicator, and a method of collecting monetary donations for Donate Life California.

Subscribe to the new E-Mail Alert Service by logging on to the department's Internet website at www.dmv.ca.gov and navigating to Vehicle Industry & Commercial Permits> (Industry Information) Vehicle/Vessel Registration Procedures. Select the Subscribe to E-Mail Alerts button and follow the simple instructions. The new service, free to DMV's industry partners, banks, and financial institutions, will alert you immediately when new memos or publications are placed on the DMV Internet website.
