All-Terrain Vehicle Safety Training Organization Requirements
An application consists of the following forms:
- OL 29, Application for Occupational License Personal History Questionnaire, Part B, from each person listed on form OL 216.
- OL 133, All-Terrain Vehicle Safety Training Organization Insurance Certificate
- OL 216, Application for Occupational License
- OL 218, Surety Bond of Driving School Owner or All Terrain Vehicle Safety Training Organization ($10,000) including Certificate of Appointment, on reverse side of bond.
- DMV 8016, Request for Live Scan Clearance (receipt) - Required of each person listed on form OL 216.
The following documents are also required as part of the application package:
- Corporation, Limited Liability Company or Limited Liability Partnership owned businesses only: A copy of Articles of Incorporation, Corporate Minutes or other document filed with the Secretary of State, which identifies the officers, share holders and managers with 10% or more interest in the business is required.
- Copy of your City and/or County Business License.
- Copy of your Fictitious Name Statement
- Copy of your rental or lease agreement

All-Terrain Vehicle Safety Training Organization Licensing Fees
The fees to apply are:
- $150 Non-refundable application
- $1 For Family Support Program
- $42 Per owner submitting fingerprint card in-lieu of livescan receipt
- $70 For each branch license or license modification, or change in corporate structure
- $15 Duplicate or replacement license
- $50 Annual renewal fee
All-Terrain Vehicle Safety Training Certificates are:
- $75 Book of 25 student licenses (3.00 each)
An application must be completed within one year or a new application with all examinations and fees will be required. The Family Support Program fee is paid on original, renewal, and reinstatement applications.
All-Terrain Vehicle Safety Instructor's Licensing Fees
The fees to become an all-terrain safety instructor are:
- $30 Non-refundable application
- $1 For Family Support Program
- $15 Duplicate or replacement license
- $15 Transfer of license (to another organization)

- ADM 1316, Fingerprint Card (out-of-state applicants only) Fingerprint cards may be obtained by calling Occupational Licensing at (916)229-3126 or your local Inspector.

The following documents are also required as part of the application package:
- Certificate from All-Terrain Vehicle Safety Association confirming a passing score for an Instructor Written Test and Skill Test.
