Driving School and Traffic Violator School Complaints
General Requirements: All complaints must be in writing when filing a complaint.
For a complaint against either a Driving School or a Traffic Violator School use the OL 620, Driving School and Traffic Violator School Complaint & Instructions form. For all other licensing complaints, use the INV 172A, Record of Complaint form.
The OL 620 can be obtained by calling the Driving School and Traffic Violator School Complaint Unit at (916) 229-3154 or by visiting your nearest DMV field office.
If a Complaint Form Is Not Available
A complaint will be accepted in letter form; however, when using a letter format please provide the following:
- Your name, mailing address, and phone number.
- Who is your complaint against?
- What is your complaint (please be specific)?
- When did your complaint/issue(s) occur?
- Please provide all applicable time(s) and date(s).
- Where did the complaint situation(s) occur?
- Also, please indicate what you would like the department to do regarding your complaint.

Mail all other license complaints to the DMV Investigations Office closest to where the incident took place as listed on the INV 172A, Record of Complaint form.
