How to Complete the Application for Occupational License Part C OL 21

Instructions

1. Fill out name of individual, partners, or corporation.

2. Fill out the telephone number of the dealer.

3. List the dealer’s name (Doing Business As),

4. List the address, city, zip code, and county of your dealership.

5. Check the appropriate box for the type of license you desire.

6. Check the appropriate box for "Reason for Submission".

Note:Sections A and B do not apply to you at this time.

7. Fill out the property data – check the appropriate box.

8. Check the approximate box, indicating whether your dealership’s location meets zoning requirements by either the city or the county.

9. List property owner’s full name, address, city, and telephone number.

10. If a new vehicle dealer, check the appropriate box.

11. Enter the county where you are completing this application.

12. Date the application.

13. Sign your name.

14. Sign your title.

Do Not complete the reverse side of this application. It is for Departmental personnel to complete when your location is inspected.
