Vehicle Transporter License Forms
An application for a vehicle transporter license consists of the following forms:
- OL 12, Application for Original Occupational License, Part C
- OL 21A, Original Application for Occupational License, Part A
- OL 29, Application for Occupational License Personal History Questionnaire, Part B (Required for each person listed under ownership on form OL 12.)
- ADM 1316, Fingerprint Card (out-of-state applications only) Fingerprint cards may be obtained by calling Occupational Licensing at (916) 229-3126 or your local Inspector.
- ADM 9050, Appointment of Director as Agent for Service of Process
- DMV 8016, Request for Live Scan Clearance (receipt) (More Information about Live Scan) (Required for each person completing form OL29.)
The following documents are also required as part of the application package:
- Photograph(s) of business location, refer to Photograph Procedures.
- Corporation, Limited Liability Company or Limited Liability Partnership owned businesses only: A copy of Articles of Incorporation, Corporate Minutes or other document filed with the Secretary of State, which identifies the officers, share holders and managers with 10% or more interest in the business is required.
Additional Items
The following items may be required by other agencies:
A Permit from the Department of Transportation (CALTRANS) for movement of over wide objects.
- State transporters are also licensed by Public Utilities Commission
- Interstate transporters are also licensed by U.S. Interstate Commerce Commission.
"Place of Business" Inspection. After you have arranged for an appointment with an Occupational Licensing Inspector he/she will conduct an inspection to ensure:
- The principal place of business, is the location of record with the department.
- If the mailing address of the licensee is not at the actual principal place of business, such mailing address shall be reflected in the application along with the address of the principal place of business and thereafter remain current with the department.
- If the principal place of business is not located in this state, the department may require such supporting documentary proof as it deems necessary to determine the applicant's status as an actual transporter pursuant to Vehicle Code Section 320. CCR, Title 13, Section 295.00 (a) (b) (c)
Note: Place of business inspection is within California only.
Original Application Fees
- $100 Non-refundable application fee
- $1 Family Support Program fee
- $50 For each branch location (if applicable)
- $70 For each auto plate (plus county fees, if applicable).
The Family Support Program fee is paid on original, renewal, and reinstatement applications.
