Request for Vehicle Vessel Automated Record Information INF 1123

The INF 1123 form:

  • is used when a Commercial Requester Account Re-Seller submits a request for vehicle/vessel information from the Department of Motor Vehicles (DMV) on behalf of another approved Requester Account End-User.
  • must contain the Commercial Requester Account Re-Seller Requester Code, and Agreement numbers issued by the DMV.
  • must contain the Commercial Requester Account End-User’s Requester Code and Agreement numbers issued by the DMV.
  • must be submitted in duplicate.
  • must be submitted to the address on the form or the address listed below.

DMV cannot accept forms submitted by e-mail at this time. Forms must be:

  • printed and completed by hand, or
  • printed and completed by typewriter, or
  • completed on-line and printed.
