Request for Record Information

You can get a copy of your DMV driver license/identification card (DL/ID) record or vehicle/vessel registration (VR) record.  Certified records are only issued from DMV Headquarters in Sacramento.

Driver records will show all reportable information. Convictions for 3, 7, or 10 years, department actions, and accidents will show on a person’s driver record as required by CVC §1808. Questions?  See Driver Record Request FAQs.

The Request for Record Information (INF 70) PDF form will allow you to get a copy of your own photograph, a copy of a deceased person’s photograph if you are an immediate family member with a certified copy of a death certificate, or a person's DL/ID or VR record information; other than your own.

Each request is reviewed to make sure that the reason of requesting the information is valid.

  • Fill out the INF 70 form. For a certified copy, mark the box provided.
  • Include the appropriate fee for each record by check or money order.
  • Mail the completed form and check or money order the address provided on the form.

To get your own record fill out the Request for Own Driver License or Vehicle Registration Record (INF 1125) (PDF).
Each request is reviewed to make sure that the reason of requesting the information is valid.

  • Fill out the INF 1125 (PDF) form. For a certified copy, mark the box provided.
  • Include a $5 fee for each record by check or money order.
  • Mail the completed form and check or money order the address provided on the form.
