Initial Requirements:
- Lesson Plan Approval - Submit one of the following:
- OL 764, Request for Approval of TVS Educational Program (PDF) with the following:

- Fee for the original review of
Classroom Course: $475 Home Study: $800 Internet Course: $800 -
- OL 766, Request for Use of an Approved TVS Program with the following attached:
- A letter from the program owner authorizing you to utilize a previously approved lesson plan.
- Fee of $25.00
Submit your request to the department at the address below. Allow a minimum of 120 days for a response. Incomplete requests will be returned. Lesson Plans will be logged and referred for approval in the order received.

An application for a traffic violator school consists of the following forms:
- OL 29B, Application for Occupational License Personal History Questionnaire, Part B Business Licensing Unit (required for each person listed on form OL 760)
- OL 140, Property Use Verification for a Driving School or Traffic Violator School License
- OL 142, Traffic Violator School Owner Assurance of Compliance With the Americans with Disabilities Act (ADA) of 1990
- OL 144, Traffic Violator School Classroom Lease or Rental Agreement
- OL 704, Surety Bond of Traffic Violator School (TVS) Owner ($2,000 ) OR OL 704 B, Traffic Violator School (TVS) Owner Surety Bond (PDF) OR OL 25E, Deposit Agreement and Assignment (PDF)
- OL 713, Traffic Violator School (TVS) Owner License, Part A
- OL 712, Traffic Violator School Branch Business Office/Classroom Application
- ADM 9050, Appointment of Director as Agent for Service of Process

Required for out-of-state applicants only and may be obtained by calling Occupational Licensing at (916) 229-3126 or your local Inspector.
The following documents are also required as a part of the application package:
- Photograph(s) of business and/or classroom location, refer to Photograph Procedures
- A completed OL 764 or OL 766 and a letter signed by DMV or an agent of DMV approving your lesson plan or authorizing you to utilize a previously approved lesson plan.
- Copy of your rental or lease agreement for all offices and classrooms.
- If filing as a Corporation, Limited Liability Company or Limited Liability Partnership owned businesses only: Statement of Information (SI500) or Statement of Information LLC (LLC 12).
Traffic Violator School Program Fee Schedule
An application must be completed within one year or a new application, all examinations, and fees shall be required. (Note: fees are good for three (3) test attempts or one year.)
The Family Support Program fee is paid on original, renewal, and reinstatement applications.
The license to own a school does not include the right to give instruction or perform operator responsibilities.
It may be more than 60 days before a school name appears on the hardcopy TVS List distributed by Traffic Assistance Providers pursuant to CV Section 11205 (a) (b).

- INFORMATION SECURITY & DISCLOSURE STATEMENT PUBLIC/PRIVATE PARTNERSHIPS EMPLOYEE (EXEC 200X REV3/2003)
- Submit Access Enrollment forms #1 and #2 above, for Primary Administrators per instructions on the forms.
- DMV will establish your primary account, and contact you via e-mail with instructions to finalize the enrollment process, and how to establish user accounts for your court or TVS users.
- User Instructions and Court Code Lists are available after a user has logged into the TVCC application.

Note: Forms #1 and #2 are required for enrollment, and instructions are printed at the top of each form. Only one set of forms per traffic school or court will be accepted. Form #3 above must be retained by the Traffic School or Court with employee records and must be updated annually.
(IMPORTANT: Each user must have a unique e-mail account to be granted access, and access will only be provided to one license or one court at a time.)
