Obtaining Information From DMV Records

Statistical Information

DMV maintains information on approximately 36 million vehicle/vessel registrations (VR), 30 million driver licenses (DL) and/or identification (ID) cards, and 420,000 occupational licensing (OL) records.

Confidential information is not considered public record. This includes certain DMV personnel matters, physical/mental information, residence address, social security number (SSN), incomplete findings from research, results of ongoing investigations, operation plans, and electronic data security controls.

Residence address and SSN are confidential except when the requester is authorized by law to receive it.

How to Obtain Information

To request public information from DMV, submit the appropriate form and fee online, by mail, or visit a local field office. Fees are nonrefundable. Fees submitted by mail must be paid by check, cashier’s check, or money order.

How to Obtain Forms

Order Forms in Bulk Quantities

Bulk orders (quantities greater than 15) are processed by DMV Materials Management Section by:

All requests must include:

  • Business name, telephone number, contact name, and street address for delivery. Delivery will not be made to a PO Box.
  • Form number, title, and quantity requested.

What Form to Submit

The form and fee are determined by the requested information. Forms must be completed online and printed (preferred), typed, or completed by hand. Below is a description of the most common requests:

Online Requests

You can request your own DL/ID or VR record for $2. This record is not an official document. Prior to submitting the request, you must create an online account. Visit dmv.ca.gov for additional information. If you need an official document, you must submit a (INF 1125) or (INF 70) form with $5 by mail or at your local field office.

Kiosk Requests

Kiosks can process and immediately print a request for your own DL/ID or VR record for $5.

Office Requests

Field offices can process a:

  • Request for Your Own Driver License/ Identification Card (DL/ID) or Vehicle/ Vessel Registration (VR) Information Record (INF 1125).
  • Request for Record Information (INF 70).
  • Registration Information Request for Lien Sale (INF 1126).
  • Dealer Vehicle/Vessel Registration (VR) Information Request (INF 1120).

Driver Safety (DS) Hearing Transcripts Requests (Administrative Records/ Transcripts) and DS Hearing Process

Requests for procedures governing DS hearings (also available free at dmv.ca.gov) must be submitted in writing with a $2 nonrefundable deposit fee.

Hearing transcripts may be requested from the department. If the requester is not the driver or driver’s representative, any personally identifiable information/confidential information will be masked (redacted).

CDs of audio recordings may be requested by the driver or their representative. As the recordings contain personally identifiable information/confidential information that cannot be redacted, they are not available for public release.

Requests for a copy of a DS document and/or audio file must be submitted in writing with the appropriate fee(s).

Financial Responsibility Requests

To request financial responsibility information, an uninsured motorist certificate, or copy of a DMV Report of Traffic Accident Occurring in California (SR 1) form, submit a completed Financial Responsibility Information Request (SR 19C) form and $20 to:

Accident and insurance information is only released to individuals or an attorney for an individual involved in an accident.

Occupational Licensing (OL) Requests

Information about a car dealer, salesperson, dismantler, driving school, registration service, traffic violator school, traffic violator school list, or vehicle verifier is available free of charge when using the Occupational Licensing Status Information System. Information includes license status, ownership information, and history of disciplinary actions taken by DMV.

To request a current OL printout not available on the Occupational Licensing Status Information System, you must submit a completed Request for Occupational Licensing Information (OL 100) form with $5 to:

To request a copy of an OL record (applications, bond documents, etc.), submit a Public Records Act request. For additional information refer to the Public Record Act and/or Information Practices Act Requests section.

National Driver Register (NDR) Requests

Employers: To obtain information reported by a state to the NDR on a current or prospective employee, submit a completed Request for National Driver Register (NDR) File Check on Current or Prospective Employee (INF 1301A) form and $5 to:

Individuals: Submit a notarized letter directly to the Department of  Transportation NDR. The INF 1301A form is not required and there is no charge for this service. Your letter must include your full legal name, date of birth, gender, height, weight, eye color, DL number, and issuing state (your SSN is optional).

The fee for a copy of Form 700 is $.10 per page. A $5 fee is charged for a copy of Form 700 that is 5 or more years old.

Public Records Act (PRA) and/or Information Practices Act Requests

To request documents pursuant to the PRA, write a description of the kind of records you need so that DMV personnel can properly identify them. If you are involved in a lawsuit, you or your attorney may serve the Legal Affairs Division with a subpoena to get a copy of the documents.

Fees are determined by the number of copies and availability of the documents requested. DMV will tell you the final cost. You must pay the fees before DMV can release the records.
