{"id":15202,"date":"2020-04-15T15:48:40","date_gmt":"2020-04-15T22:48:40","guid":{"rendered":"https:\/\/www.dmv.ca.gov\/portal\/handbook\/vehicle-industry-registration-procedures-manual-2\/form-specifications\/acceptance-of-reproduced-forms-by-dmv\/"},"modified":"2024-09-27T11:12:01","modified_gmt":"2024-09-27T18:12:01","slug":"acceptance-of-reproduced-forms-by-dmv","status":"publish","type":"dmv_handbook","link":"https:\/\/www.dmv.ca.gov\/portal\/es\/handbook\/vehicle-industry-registration-procedures-manual-2\/form-specifications\/acceptance-of-reproduced-forms-by-dmv\/","title":{"rendered":"29.015 Reproducing DMV Forms"},"content":{"rendered":"<p>The following must be reproduced as closely as possible to the DMV version:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Size of the finished form.<\/li>\n\n\n\n<li>Format (precise placement of text, graphics, and margins).<\/li>\n\n\n\n<li>Content (latest revision).<\/li>\n\n\n\n<li>Number of pages or sides.<\/li>\n\n\n\n<li>Overall legibility must be equivalent or better.<\/li>\n\n\n\n<li>Typeset font size.<\/li>\n\n\n\n<li>Type face (Arial or equivalent).<\/li>\n\n\n\n<li>Paper weight and color of paper and ink.<\/li>\n\n\n\n<li>Form number and revision date.<\/li>\n\n\n\n<li>3\/8 inch (2 picas) blank margin around the form.<\/li>\n\n\n\n<li>Necessary indices or markings for microfilm processing.<\/li>\n<\/ul>\n\n\n\n<p>It is the requester\u2019s responsibility to check with DMV for the latest revision of the form.<\/p>\n\n\n\n<p>New Requirements:<\/p>\n\n\n\n<p>Copyright Notice\u2013must consist of the following elements:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>The word \u201cCopyright.\u201d<\/li>\n\n\n\n<li>The copyright symbol \u201c\u00a9.\u201d<\/li>\n\n\n\n<li>The year of revision or, for a new form, first year of publication.<\/li>\n\n\n\n<li>\u201cState of California, Department of Motor Vehicles.\u201d<\/li>\n\n\n\n<li>\u201cAll rights reserved.\u201d<\/li>\n<\/ul>\n\n\n\n<p>Example for REG 42 (REV. 3\/2007) form:<\/p>\n\n\n\n<p>Copyright \u00a9 2007 State of California, Department of Motor Vehicles. All rights reserved.<\/p>\n\n\n\n<p>The copyright symbol may be typed with the keyboard number lock on by pressing and holding the Alt key and typing \u201c0169\u201d on the number pad.<\/p>\n\n\n\n<p>Reproduced Form Box\u2013replaces the DMV logo and must contain the following elements:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>The box should measure .5 inch x 1.19 inches.<\/li>\n\n\n\n<li>The words \u201cReproduced Form\u201d in 8 point bold Arial font.<\/li>\n\n\n\n<li>The approval number provided by the Divisional Forms Reproduction Administrator in 12 point bold Arial font.<\/li>\n<\/ul>\n\n\n\n<p>DMV forms are subject to copyright law protections. Written permission from the DMV Divisional Form Reproduction Administrator must be obtained to reproduce any DMV Form.<\/p>\n\n\n\n<p><span role=\"region\" class=\"format-label--note\"><span class=\"format-label__text\">Note<\/span> Requests to reproduce forms with serial numbers may be considered but will be subject to additional requirements reporting criteria, and a signed agreement.<\/span><\/p>\n\n\n\n<p>The following form elements must not be reproduced:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>DMV logos.<\/li>\n\n\n\n<li>Any reference to the Office of State Publishing (OSP)\u2013either OSP or a seven or eight digit number on the bottom right hand corner of the form (i.e., 14 12345).<\/li>\n\n\n\n<li>The State Seal.<\/li>\n<\/ul>\n\n\n\n<p><strong>Exception<\/strong>: If the form is photocopied, the items listed above do not need to be removed.
