Additional REG 42 forms must be requested by submitting a Dismantlers Acquisition, REG 42 Order Form (<a href=\"https:\/\/www.dmv.ca.gov\/portal\/es\/file\/dismantler-acquisition-reg-42-order-form-ol-395d\/\" target=\"_blank\" rel=\"noreferrer noopener\">OL 395D<\/a>) to the address on the form. This form is only available online under OL supply order forms.<\/p><\/p>\n\n\n\n<p><p dir=\"ltr\"><strong>Emergency situations<\/strong>\u2014New licensees and all currently licensed dismantlers, in emergency situations only, may obtain a minimal quantity of Notice of Acquisition\/Report of Vehicle to be Dismantled (REG 42) forms over the counter at inspector\u2019s offices (see Chapter 10).
