width=\"1043\" height=\"827\" aria-hidden=\"true\" alt=\" \"\/><\/figure>\n<\/div>\n\n\n\n<div class=\"wp-block-dmv-column col\">\n<div class=\"wp-block-dmv-container shadow-box\">\n<p>Pursuant to state law, the Department of Motor Vehicles (DMV) will conduct a Statewide Ignition Interlock Device (IID) Pilot Program that requires all repeat and all injury-involved driving under the influence (DUI) offenders to install an IID for a time-period ranging from 1 to 4 years, depending on the number of prior DUI convictions.<\/p>\n\n\n\n<p>The pilot program applies to DUI offenders convicted of a DUI involving alcohol or a combination of alcohol and drugs, and whose offenses occur between January 1, 2019 and January 1, 2033.<\/p>\n\n\n\n<p>Offenders subject to mandatory IID installation under the pilot program may qualify for an exemption.<\/p>\n\n\n\n<p><strong>If an exemption is granted, offenders must serve their full suspension or revocation term and complete their DUI treatment program.<\/strong><\/p>\n\n\n\n<div class=\"wp-block-dmv-spacer-block spacer-box\"><\/div>\n\n\n\n<h2 class=\"wp-block-heading\">IID Program Exemption<\/h2>\n\n\n\n<p>If your violation date occurred on or after <strong>January 1, 2019<\/strong>, and you are subject to mandatory IID installation pursuant to <em>California Vehicle Code<\/em> (VC) \u00a723575.3, then you may qualify for an exemption if you meet all of the following requirements:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>You do not own a vehicle.<\/li>\n\n\n\n<li>You do not have access to a vehicle at your place of residence.<\/li>\n\n\n\n<li>You no longer have access to the vehicle used when arrested for the violation that resulted in the DUI conviction.<\/li>\n<\/ul>\n\n\n\n<p>If the above applies to you, then you must complete the following on the <a href=\"https:\/\/www.dmv.ca.gov\/portal\/file\/ignition-interlock-device-exemption-request-dl-4062-pdf\/\">Ignition Interlock Device (IID) Exemption Request (DL 4062) (PDF)<\/a> form:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Section 1 \u2013 Driver Information<\/li>\n\n\n\n<li>Section 2 \u2013 Exemption (check the <strong>IID Program Exemption<\/strong> box)<\/li>\n\n\n\n<li>Section 3 \u2013 Certification<\/li>\n<\/ul>\n\n\n\n<p>Once completed, you must submit the DL 4062 form to DMV at the e-mail address below within 30 days from the date of your suspension or revocation notice.<\/p>\n\n\n\n<div class=\"wp-block-dmv-spacer-block spacer-box\"><\/div>\n\n\n\n<h2 class=\"wp-block-heading\">Out-of-State Exemption<\/h2>\n\n\n\n<p>If your violation date occurred on or after <strong>January 1, 2019<\/strong> in California and you are subject to mandatory IID installation pursuant to VC \u00a723575.3, but you reside in another state, then you may qualify for an out-of-state exemption if the following apply:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>You <strong>have<\/strong> completed a California DUI treatment program.<\/li>\n\n\n\n<li>You <strong>have not<\/strong> completed your mandatory IID restriction period.<\/li>\n<\/ul>\n\n\n\n<p>If the above applies to you, then you must complete the following on the <a href=\"https:\/\/www.dmv.ca.gov\/portal\/file\/ignition-interlock-device-exemption-request-dl-4062-pdf\/\">DL 4062 (PDF)<\/a> form:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Section 1 \u2013 Driver Information<\/li>\n\n\n\n<li>Section 2 \u2013 Exemption (check the <strong>Out-of-State Exemption<\/strong> box)<\/li>\n\n\n\n<li>Section 3 \u2013 Certification<\/li>\n<\/ul>\n\n\n\n<p>Once completed, you must submit the DL 4062 form <strong>and<\/strong> at least one acceptable out-of-state residency document to DMV at the e-mail address below.<\/p>\n\n\n\n<p>An acceptable out-of-state residency document is one of the following documents that lists your first and last name, and the residence address matches the residence address listed on the DL 4062 form:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Receipts for payment of resident tuition at a public institution of higher education or school records.<\/li>\n\n\n\n<li>Home utility bill including cellular phone bill.<\/li>\n\n\n\n<li>Documents issued by a licensing authority indicating a current application for a driver license in the state of residence.<\/li>\n\n\n\n<li>Official voter registration documents.<\/li>\n\n\n\n<li>Official document issued by a governmental agency that can be used by the department to prove residency.<\/li>\n\n\n\n<li>Rental or lease agreement with the signature of the owner\/landlord and the tenant\/resident.<\/li>\n\n\n\n<li>Deed or title to residential real property.<\/li>\n\n\n\n<li>Mortgage bill.<\/li>\n\n\n\n<li>Filing of homeowner\u2019s property tax exemption or homestead exemption certificate.<\/li>\n\n\n\n<li>Medical document.<\/li>\n\n\n\n<li>Employment document.<\/li>\n\n\n\n<li>Insurance document, including medical, dental, vision, life, home, rental, and vehicle.<\/li>\n\n\n\n<li>Change of Address Confirmation by the United States Postal Service.<\/li>\n\n\n\n<li>Property tax bill or statement.<\/li>\n\n\n\n<li>Faith based document that includes the name and address of the issuing organization.<\/li>\n\n\n\n<li>Tax return documents.<\/li>\n\n\n\n<li>Record of financial institution.<\/li>\n\n\n\n<li>Court documents that list the applicant as a resident outside of California.<\/li>\n<\/ul>\n\n\n\n<p>Additional information can be found in the <a class=\"wp-block-dmv-spacer-block spacer-box\"><\/div>\n\n\n\n<h2 class=\"wp-block-heading\">Medical Exemption<\/h2>\n\n\n\n<p>If your violation date occurred on or after <strong>January 1, 2019<\/strong>, you are subject to mandatory IID installation pursuant to VC \u00a723575.3, and you have a medical condition that does not permit you to breathe with sufficient strength to activate the IID, then you may qualify for a medical exemption.<\/p>\n\n\n\n<p>If the above applies to you, then you and your medical provider must complete the following on the&nbsp;<a href=\"https:\/\/www.dmv.ca.gov\/portal\/file\/ignition-interlock-device-iid-medical-exemption-request-dl-4063-pdf\/\">Ignition Interlock Device (IID) Medical Exemption Request (DL 4063) (PDF)<\/a>&nbsp;form:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Section 1 \u2013 Driver Information (completed by driver)<\/li>\n\n\n\n<li>Section 2 \u2013 Acknowledgement and Consent (completed
