Traffic Violator School Owner License Forms and Fees

Initial Requirements:
  1. Lesson Plan Approval - Submit one of the following:
    1. A letter from the program owner authorizing you to utilize a previously approved lesson plan.
    2. Fee of $25.00

Submit your request to the department at the address below. Allow a minimum of 120 days for a response. Incomplete requests will be returned. Lesson Plans will be logged and referred for approval in the order received.

2. OL 612, Request for DMV Approval of Traffic Violator School Name.

    • You may call (916) 229-3126 to confirm name availability before completing this form.
    • Follow instruction on the form
    • No fee is charged to reserve a proposed name for six months.
An application for a traffic violator school consists of the following forms:

The following documents are also required as a part of the application package:

  • A completed OL 764 or OL 766 and a letter signed by DMV or an agent of DMV approving your lesson plan or authorizing you to utilize a previously approved lesson plan.
  • A copy of your Business License
  • Copy of your Fictitious Name Statement
  • Copy of your rental or lease agreement for all offices and classrooms or use form OL 144, Traffic Violator School Classroom Lease or Rental Agreement.
  • Corporation, Limited Liability Company or Limited Liability Partnership owned businesses only: A copy of Articles of Incorporation, Corporate Minutes or other document filed with the Secretary of State, which identifies the officers, share holders and managers with an interest in the business is required.

An application must be completed within one year or a new application, all examinations, and fees shall be required.  (Note:  fees are good for three (3) test attempts or one year.)

The Family Support Program fee is paid on original, renewal, and reinstatement applications.

The license to own a school does not include the right to give instruction or perform operator responsibilities..

      3. INFORMATION SECURITY & DISCLOSURE STATEMENT PUBLIC/PRIVATE PARTNERSHIPS EMPLOYEE (EXEC 200X REV3/2003)
      4. Submit Access Enrollment forms #1 and #2 above, for Primary Administrators per instructions on the forms.
      5. DMV will establish your primary account, and contact you via e-mail with instructions to finalize the enrollment process, and how to establish user accounts for your court or TVS users.
      6. User Instructions and Court Code Lists are available after a user has logged into the TVCC application.
      7. E-mail LOD.TVS@dmv.ca.gov or call (916) 657-6550 for assistance.


      8. Note: Forms #1 and #2 are required for enrollment, and instructions are printed at the top of each form. Only one set of forms per traffic school or court will be accepted. Form #3 above must be retained by the Traffic School or Court with employee records and must be updated annually.

(IMPORTANT: Each user must have a unique e-mail account to be granted access, and access will only be provided to one license or one court at a time.)
