Request for DUI Program Forms (DL 101R)

DUI program Signatory Authority (DL 101S)

This form is used by DUI Program owners/directors to add or delete authorized employees for the purpose of both ordering and signing the DUI Program forms, specifically the DL 101 and the DL 107.

Request for Dismissal of Failure to Appear (FTA) Violations (DL 114)

This form requests the dismissal of failure to appear FTA violations.

Request for Cancellation or Surrender of a Driver License or Identification Card (DL 142)

This form requests the cancellation or surrender of an individual's driver license or identification card.

Driver License Record Correction Request (DL 207)

Use this form to dispute an incorrect traffic violation or conviction information on your driving record, other than motor vehicle traffic accidents. You must submit certified court abstracts an/or court documentation in order for the DMV to update your record.

Private Secondary Non-Classroom Driver Education Curriculum Certification (DL 396B)

This form is an application for the certification of private secondary non-classroom driver education curriculum.

Report Out-of-State Traffic Conviction by a Commercial Driver (DL 535)

This is a form to report a commercial driver for having a traffic convictions outside of the state of California.

Physician’s Health Report (DL 546A)

This is for drivers requesting a 45-foot housecar endorsement and individuals applying for a driving instructor license. It is required for these individuals. All original applicants must submit, at the time of the application and every two years thereafter. An applicant who has a Health Questionnaire DL 546 on file must also submit a Physician's Health Report DL 546A. Exception: An applicant who is required to have and maintains a current Medical Examination Report (MER) form MCSA 5875 and Medical Examiner’s Certificate (MEC) form MCSA 5876, will be excluded from this requirement.

Report of Vision Examination (DL 62)

The applicant takes this form to an opthalmologist or optometrist. The applicant must complete the top portion of the DL 62 report form, including signing and dating the authorization line. The applicant may complete the top portion of the DL 62 online and print the form for the vision specialist who will provide applicable information in the remaining sections of the form. It is the applicant's responsibility to return the completed form to DMV. Do not mail this form back to DMV unless directed to do so by a DMV employee.

Participant’s Certification of DUI Program Enrollment or Completion (DL 804)

When the program provider is unable to capture the participant's signature on the paper version of either the DL 101 or DL 107 certificate, or if the provider is submitting an electronic certificate, the DL 804 must be used to capture the program participant's signature, certifying under penalty of perjury that the participant has either enrolled in or completed the required DUI program. This form is to be completed and used only by a licensed DUI Program Provider.
