Business Partner Automation Program Information Security Pre-Implementation Checklist (EXEC 5555B)

This form is to assure that the applicants have met the Departments minimum-security standards before they are approved for participation in the Business Partner Automation program. This form must be submitted in conjunction with the Second-line business partner application.

This form is used primarily by dealers/dismantlers and also individuals or companies submitting multiple registration applications to DMV. Three copies of the transmittal form must be submitted to DMV.

This form is used to apply for a Commercial Request Account, during renewal of your requester code and when any changes occur to the company name, address, phone number, or contact person. It must be submitted by the contact person on file within 10 days of occurrence to avoid delays in receiving requested information, or to avoid possible closure due to the DMV receiving returned unclaimed mail.

Request for Vehicle/Vessel Photo History Information (INF 1124)

The INF 1124 form is used by approved Commercial Requester Account holders to request Vehicle/Vessel Photo History Information. Indicate whether the photo history must be certified (for Court purposes). Three (3) years plus current are available, not all records have photo histories available, while some may have additional years.
