Statement to Record Ownership (REG 101)

The Statement to Record Ownership is used when the new owner sections of the title are full or contain incorrect information. The Statement of Error or Erasure is used to identify information that has been written on a Certificate of Title/Ownership in error.

Certificate of Repossession (REG 119)

Side A- This form is used by a vehicle's legal owner to explain when, why, from whom, and by whom the vehicle was repossessed. Side B- This form is used by the legal owner to record the conditional sale by transfer, auction, or private sale of a repossessed vehicle or vessel.

Notice of Transfer and Release of Liability (REG 138)

When the owner of a California registered vehicle sells or transfers title or interest in the vehicle, the seller must complete a Notice of Transfer and Release of Liability (REG 138) and submit it to the department within five calendar days. Keep a copy of the REG 138 for your own records.

Vehicle Emission System Statement (SMOG) (REG 139)

This form is used to establish agreement about the rights and responsibilities of both the buyer and seller of a vehicle regarding smog certification.

Lien Satisfied/Title Holder Release (REG 166)

This form may be used by the legal owner/lienholder of record instead of the Certificate of Title to release interest in a vehicle. The form must be notarized if it will be submitted with an Application for Duplicate California Title.

Certification of Lien Sale (REG 168)

This form is used to certify that a vehicle valued over $4000 has undergone the lien sale process including the 10-day redemption period.

Certification of Lien Sale for Vehicle Valued $4000 or Less (REG 168A)

This form is used to certify that a vehicle valued $4000 or less has undergone the lien sale process.

Application for Replacement or Transfer of Title (REG 227)

This application form is used for a duplicate title, paperless title certification, transfer of title with duplicate title, or transfer of title with paperless title. You can make an appointment at your local DMV or mail the completed forms with payment.

Notice of Pending Lien Sale of Vehicle Valued over $4000 or From a Self-Service Storage Facility (REG 280)

This form is sent by the lienholder to the owner(s) of record, interested parties, and the department at least 20 days prior to the lien sale as notice of the pending sale.
