Application for Title or Registration/Verification of Vehicle (REG 343)

This application form is used for vehicles being registered for the first time and for nonresident vehicles brought into California. It must be submitted with the appropriate fees within 20 days of the date the vehicle is first operated in California. This form may also be used to reregister a vehicle after it has been junked, salvaged, or removed from the department's database. The REG 343 is generally submitted with other supporting documents and is not to be mailed by itself.

Permanent Trailer Identification Certification (REG 4017)

The REG 4017 is a two-sided form used for trailers in the PTI program. Side A is completed and signed by sole owners requesting a paper certificate of title. Side B is an application for: the transfer of ownership for a PTI trailer with an electronic title record or addition of a legal owner only. The transfer may be a registered owner transfer or a release of security interest only.

Unobtainable Title Certification for Issuance of Salvage Certificate (REG 492)

Affidavit for Transfer Without Probate California Titled Vehicle or Vessels Only (REG 5)

May be used under certain conditions to transfer ownership of a vehicle when the registered owner or legal owner (an individual) of the vehicle is deceased.

Courtesy Stop Request (REG 500)

This form is used to request a Vehicle License and Titling (VLT) Courtesy Stop be placed on a vehicle record.

Vehicle, Vessel, or Off-Highway Vehicle Title Deposit Agreement and Assignment (REG 5059)

This form is an agreement used by an applicant, and if applicable, financial institution, to post a deposit in-lieu of a bond in an amount equal to the value of a vehicle, vessel or off highway vehicle for which documentary evidence of ownership is requested when the applicant is unable to provide the regularly written required evidence of ownership.

Notification of Alternative Forms of Financial Responsibility (REG 5085)

This form must be completed when a vehicle covered by a commercial/business insurance policy or an alternative form of financial responsibility such as a cash deposit, bond, or self insurance is transferred to a new owner or registered for the first time in California. The REG 5085 must be submitted with the titling documents.

Application for Authorization for Lien Sale of a Lien Sale for a Vehicle Valued Over $4000 (REG 656)

This form is used to apply for authorization to conduct a lien sale for a vehicle valued over $4000.

Application for Authorization to Continue Lien Sale After Unsuccessful Service (REG 659)

This form is used after an unsuccessful service of the notice of pending lien sale of a vehicle valued at $4000 or less. The notice of pending lien sale is sent to the registered owner(s), legal owner/lienholder, if any, other known interested parties, and DMV prior to the lien sale.

Notice of Pending Lien Sale for Vehicle Valued $4000 or Less

This form is used by lienholders to notify interested parties of a pending lien sale.
