We value your feedback regarding your experience with autonomous vehicles operating on California public roads. There are two ways to report an autonomous vehicle incident or safety concern.
Report to the California DMV
Use the AV Incident Form to submit feedback or report an incident involving an autonomous vehicle operating on California public roads.
Please do not include sensitive personal information such as your Social Security number, driver’s license number, or financial account number.
Report to the National Highway Traffic Safety Administration
To report a safety complaint involving an autonomous vehicle to the National Highway Traffic Safety Administration (NHTSA), use the Automated Vehicle Complaint Form.
You may also report a safety complaint by phone by calling NHTSA’s Vehicle Safety Hotline at 1-888-327-4236.
