Before you begin:

If this is for an original Disabled Person Placard application, please ensure you have completed an Application for Disabled Person Placard or Plates (REG 195).
- There is no fee for a Permanent Disabled Person Placard
- The fee for a Temporary Disabled Person placard is $6

If this is for a replacement Disabled Person Placard, please ensure you have completed an Application for Replacement Plates, Stickers, Documents (REG 156).
- There is no fee for a Permanent Disabled Person Placard
- The fee for a Temporary Disabled Person placard is $6
Please ensure:

- For an original Disabled Person Placard, have a licensed physician, surgeon, chiropractor, optometrist, physician assistant, nurse practitioner, or certified nurse midwife that has knowledge of the disease and/or disability sign the Medical Provider’s Certification section of the REG 195.
- You are ready to pay fees via eCheck or credit card for a replacement Disabled Person placard.
- A copy of an acceptable proof of true full name and date of birth including:
- A state-issued driver license (DL) or identification (ID) car
- United States (U.S.) birth certificate
- Valid and unexpired U.S. passport or U.S. passport card
- Additional birth date/legal presence documents acceptable as proof of true full name and date of birth, if needed:

If this is for a replacement Disabled Person placard, the placard will be mailed to the address on record. If you change your address online, please wait approximately two weeks before submitting this request to ensure your record was updated.
